Ok, I need help!!

I have a DVD (9 Minutes) that I want to add to my website.

When an individual visits this website I want the video to play immediately. After the DVD plays I want a page to appear that will have a form that they must complete to get more information.

Can someone please tell me where to begin?

First you should convert it to another movie format...

Then you might be able to integrate it with a flash script that'll handle the automatic playing.
