I'm on EU server and can't see the questionnaire anywhere where is the link to it? This is what I see in community homepage.

Well that was a waste of time. It dosen't say my game id in the url of reward shop, community page or mgo site.

No not your character name your Game ID. If you can't remember then go onto your PS3 and log onto your character and go to reward shop or community support page. As the page loads look at the URL and you should see your Game ID as it loads.

As in your character name? Is there anyway to find out what your info is?

What ID is it they want to sign it to that site? Is it the Konami ID?

Game ID not Konami ID, if you can't remember check your E-mail. The Game ID is the one with the password with only numbers and no letters if that helps you remember.
