I need a TOS script it needs to have the following features.

Person scrolls options the tos as if they read it then becuase of this the check box appears <(Option) along with a place for filling in there full name.

After this a visual code verification entry.

Once they have clicked the submit, all information gets entered into a mysql data base.

An email gets sent off to them with all info they entered. Then they must click a link to verify there email and fruther verify all data entered.

I would prefer a web based install option php/mysql driven such as wordpress is. It must have the data base option. If you dont know any please post keywords to look for along with suggestions on what could be used instead.

I don't think that there is something like the one you ask, but it is not too difficult to code it. Maybe you could pay about $50 to a coder to do it for you.
